Webinar: Adolescents Living with HIV - The Toolkit for Transition of Care and Other Services

PEPFAR, in collaboration with AIDSTAR-One, is presenting a series of webinars targeting program planners and policy makers interested in improving services for adolescents living with HIV (ALHIV) through use of tools supported by PEPFAR and USAID's Africa Bureau.
Please join us for the second discussion, a presentation on the AIDSTAR-One Toolkit for Transition of Care and Other Services for Adolescents Living with HIV and the accompanying Training Manual. As the number of vertically infected adolescents living with HIV (ALHIV) continues to grow, there is an increased need to support these individuals as they transition from pediatric to adult care. With the limited number of health and community care providers throughout much of sub-Saharan Africa, it is likely that many adolescents will not experience a physical transition from one clinic to another; however, all ALHIV undergo a mental transition to adulthood, and during this time-period self-care and self-management of HIV is key. Transition is a "multifaceted, active process that attends to the medical, psychological and educational or vocational needs of adolescents as they move from child focused to adult focused health care" (Reiss and Gibson 2002).
Through instruction and tools, the Toolkit for Transition of Care and Other Services for Adolescents Living with HIV provides clear guidance to community care providers (CCPs) and health care providers (HCPs), the adolescent, and his or her family/caregiver to promote a smooth transition for the adolescent. The toolkit and accompanying training manual address the topics of transition; psychosocial development; mental health; beneficial disclosure; protection; sexual and reproductive health; alcohol and substance use; loss, grief, and bereavement; clinical considerations; positive living; and linking health and community services to support ALHIV. CCPs and HCPs who use this toolkit can work with the adolescent and his or her family/caregiver to develop a package of services that are individually tailored to meet the needs of the adolescent in a developmentally appropriate manner.
This webinar will present information on the transition needs of adolescents living with HIV and outline how the toolkit may be used to help the HCP, CCP, family/caregiver and the adolescent address the various topics that arise throughout the transition period in order to promote self-management of care. The accompanying training manual, which provides technical guidance and information on the toolkit, will also be presented along with a case study to identify the specific needs of an ALHIV and how their transition needs may be addressed through utilization of the toolkit in practice. During the discussion, participants will be encouraged to share additional practices and experiences.
